We understand insurance can be confusing, and while we do our best to help our patients and customers navigate their insurance benefits, it is impossible for us to know all the in's and out's of your constantly changing insurance plan. As a courtesy to our patients we can submit to your insurance on your behalf for your exam services. However, it is ultimately the responsibility of the patient to know their insurance and what their out of pocket cost would be. Many times we are able to access your benefits and eligibility online for your vision plan benefits. We participate with the following major insurance carriers and more. As a note of importance, while we participate with most major medical carriers and three top vision insurance carriers, not all plans within those carriers are In-Network with us. If your insurance carrier isn't listed below, please call us for further details or contact your insurance provider directly.
If you have a funded vision benefit plan it will have a different name than your medical insurance. We will need the benefit company name, any identifying policy numbers and the name and date of birth of the primary policy holder. Unlike medical insurance plans, most vision plan benefits can be easily accessed and submitted online.
